DUE February 15th

 Applicants for “Teams That Make a Difference” must submit five (5) copies of a 1-to-2 page double-spaced summary as to why their team makes a difference.  In addition to the summary, applicants are to submit a one-page letter of support from the building-level administrator. (Note: Any additional materials submitted will not be considered by the committee.)  Applicants must be current AAMLE members. 

 A “team” is defined as an identified group of adults (core, elective, and/or support staff) working together and striving to address emotional, social, physical, and cognitive needs of adolescents using current middle-level practices.

 Each application will be evaluated based on the following criteria and point system:

       1)      Description of the team’s vision with defined      outcomes for students
              (10   points);

2)      Description of the team’s identity with themes, discipline 
  procedures, incentive programs, and parent/community 
  involvement, etc. highlighted (20 points);

3)      Description of the team’s uniqueness including any activities or 
  projects    that set them apart (30 points); and a

4)      Description of the how the team meets the diverse needs  
  of the student population (40 points).
